PAYOR AGREES THAT CREDIT CARD PAYMENTS, AUTOMATIC BILLING, INSTALLMENT PLAN and/or ACH TRANSACTIONS TO THE LAW OFFICE OF HARVEY F. SIEGEL ARE SUBJECT TO THE FOLLOWING TERMS AND CONDITIONS:

1. Payor acknowledges that payment(s) to payee as specified by payor below are offered to induce payee to provide legal services,  in lieu of payment in cash or certified funds.

2. Payor acknowledges that this payment authorization shall serve as payor's authorization and irrevocable letter of instruction to payor's financial institution to pay the Law Office of Harvey F. Siegel (payee) with payor's credit card, debit card or from payor's account(s), as specified by payor below.

3. Payor acknowledges that if a payment should be dishonored or declined by payor's financial institution, for any reason, payee will be correct to presume that payor's offer of payment herein was provided to fraudulently induce payee to provide services. Should the amount remain unpaid for more than seven (7) days, payor shall be responsible (criminally and civilly) for having committed fraud and larceny against payee.

4. Payor acknowledges that the Law Office of Harvey F. Siegel has afforded payor the courtesy to pay by method specified by payor and in lieu of payment in cash. Accordingly, this method of payment does not confer upon payor any rights that payor would not have had, had this payment been made in cash or certified funds.

5. Payor understand that, if payor is making payment(s) on behalf of a client other than payor, payor shall have, by virtue of said payment, no additional right(s) concerning the litigation funded by said payment(s).
